How Aged Care Physiotherapy Pty Ltd handles personal information and health information collected through this website and related enquiries.
1. Who we are
Aged Care Physiotherapy Pty Ltd (ACP) provides physiotherapy and related services in Melbourne and surrounding areas. In this policy, “ACP”, “we”, “us” and “our” refer to Aged Care Physiotherapy Pty Ltd.
For privacy enquiries, contact contact@acphysio.com.au.
2. Information we collect
Depending on how you interact with us, we may collect information such as your name, contact details, suburb or location, organisation or facility, service requested, funding pathway, referral information and information you include in a message.
Our referral forms may also collect information about a client or resident, including date of birth, reason for referral, urgency and clinical information. This may constitute health information or other sensitive information under Australian privacy law.
Our careers form may collect professional information such as profession, AHPRA registration details, experience, availability and a CV.
3. How we use information
We use information collected through this website to respond to enquiries, process referrals, discuss or arrange services, communicate with clients or their representatives, assess employment applications and administer our business.
We only ask for information that is reasonably relevant to the purpose of the form. Please do not include unnecessary health information or other sensitive information in a general website enquiry.
4. Information about another person
If you provide information about another person, such as a family member, resident or client, you should only provide information that you are authorised or otherwise entitled to provide. Where appropriate, ACP may need to confirm authority or consent before acting on a request.
5. Health information
Health information is handled as sensitive information and is used where reasonably necessary to provide or coordinate health services and respond to referrals. ACP applies appropriate administrative, technical and operational safeguards to protect information against misuse, interference, loss and unauthorised access, modification or disclosure.
6. How information is stored and processed
Website form submissions are processed through our website infrastructure and may be stored in systems used by ACP to manage enquiries and referrals. The current website uses Cloudflare for website delivery and security, Supabase for form-submission storage, and Brevo for transactional email delivery.
These providers may process information in Australia or overseas depending on their infrastructure and services. ACP takes reasonable steps to use providers and arrangements that are appropriate for the information being handled and to address applicable privacy obligations.
7. Disclosure of information
We may disclose information to people and organisations where reasonably necessary to provide or coordinate services, respond to your request, operate our business, or where required or authorised by law. This may include relevant health professionals, care providers, facilities, representatives and service providers acting on our behalf.
We do not sell personal information.
8. Website security
ACP uses HTTPS and security controls designed to reduce the risk of unauthorised access, misuse and malicious activity. Website forms use server-side validation, rate limiting and size controls. No internet transmission or storage system can be guaranteed to be completely secure.
9. Cookies, analytics and third-party services
The website may use technical functionality provided by third-party services and may receive standard technical information such as IP address, browser type, device information and request information as part of website delivery, security and operation. We do not use this information to sell personal information.
10. Access and correction
You may request access to personal information we hold about you and ask us to correct information that is inaccurate, out of date, incomplete, irrelevant or misleading, subject to applicable legal exceptions.
To make a request, contact contact@acphysio.com.au. We may need to verify your identity before providing access or making changes.
11. Privacy complaints
If you believe we have mishandled your personal information, please contact us first so we can investigate and respond to your concern.
If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner (OAIC) about a privacy complaint. More information is available from oaic.gov.au.
12. Data retention
We retain information for as long as reasonably necessary for the purpose for which it was collected, to provide services, meet professional, legal, accounting or insurance requirements, resolve disputes and protect our legitimate interests. When information is no longer required, it is securely deleted or de-identified where reasonably practicable.
13. Changes to this policy
We may update this policy when our services, systems or legal obligations change. The current version published on this page applies to information handled under the website.
