After hospitalisation
Rebuild activity tolerance, strength, walking and everyday function after an illness or hospital admission.
REHABILITATION PHYSIOTHERAPY · MELBOURNE
ACP provides practical home and community physiotherapy for older adults following illness, injury, surgery or a hospital stay. We focus on rebuilding strength, mobility, balance and confidence in the activities that matter.
RECOVERY THAT FITS REAL LIFE
Leaving hospital or finishing an inpatient rehabilitation program does not always mean a person is back to their usual routine. Walking, getting out of a chair, managing steps or returning to everyday activities can still be difficult.
ACP can assess how a person is managing at home and develop a physiotherapy plan around their current ability, goals, health conditions and home environment. Treatment is delivered within ACP's community service areas across Melbourne, subject to availability.
WHEN REHABILITATION MAY HELP
Physiotherapy may be useful when a person has lost strength, confidence or independence following a health event.
Rebuild activity tolerance, strength, walking and everyday function after an illness or hospital admission.
Progress mobility, transfers, strength and balance following a fracture, in line with medical advice and restrictions.
Support a graded return to movement and daily activities following surgery, within the treating team's recommendations.
Address reduced strength, endurance and confidence after a period of inactivity or illness.
Assess balance, gait, mobility aids and relevant falls risks, then work on practical strategies.
Work on getting out of bed or a chair, walking around the home and managing essential daily tasks.
WHAT TO EXPECT
The physiotherapist will first understand the person's current function and what they want or need to be able to do.
Review relevant history, current mobility, strength, balance, pain, transfers, walking and home environment.
Agree on realistic goals such as walking safely to the bathroom, standing from a chair or improving confidence outdoors.
Use appropriate strengthening, balance, gait practice, functional tasks and education.
Monitor response and adjust the plan as ability, health and goals change.
Depending on eligibility and individual circumstances, physiotherapy may be accessed privately, through an applicable Support at Home clinical support pathway, or through an NDIS plan where the support relates to the participant's disability and approved goals. Funding and service arrangements should be confirmed with the relevant provider or plan manager before treatment begins.
ACP can discuss the service options and explain what information may be needed for a referral.
